A kindly man came over, smiled at me and asked: &#8220;Can I help me?&#8221; Okay, so English wasn&#8217;t his first language and he meant &#8216;you&#8217; not &#8216;me&#8217;, but it got\u00a0<em>me<\/em>\u00a0thinking. How do we know where we are going, how soon we&#8217;ll arrive,\u00a0<em>when<\/em>\u00a0we&#8217;ve arrived? And what&#8217;s this got to do with top quality psychotherapy?<\/p>\n<p>When you ask someone for directions:<\/p>\n<ul>\n<li>you don&#8217;t spend hours talking about all the places you\u00a0<em>don&#8217;t<\/em>\u00a0want to go.<\/li>\n<li>you want\u00a0<em>step by step<\/em>\u00a0instructions on how to get there (some people &#8211; the best kind of direction givers) may even lead you there.<\/li>\n<li>you need to know\u00a0<em>how you&#8217;ll know<\/em>\u00a0when you&#8217;ve arrived (&#8220;you&#8217;ll see an enormous mosque by the railway&#8221;).<\/li>\n<li>it can be useful to know what indicators along the way will let you know you&#8217;re getting close (&#8220;the spice market will be on your left, the Galata Bridge on your right&#8221;).<\/li>\n<li>and when you&#8217;ve arrived at your destination you should have a much better idea how to get back there again in future,\u00a0<em>on your own if needs be.<\/em><\/li>\n<\/ul>\n<h3>Prefer to watch instead?<\/h3>\n<div class=\"video-container\"><iframe loading=\"lazy\" title=\"Watch Build Your Client A Roadmap Out Of Depression (4 Quick Psychotherapy Techniques)\" src=\"https:\/\/www.youtube.com\/embed\/TnV490u4pi0?si=M3W9usnKDWh4xHC4&amp;rel=0\" width=\"560\" height=\"315\" frameborder=\"0\"><\/iframe><\/div>\n<h3>Depression is a strange city<\/h3>\n<p>And it&#8217;s exactly the same when treating people for depression. A depressed person is essentially a stressed person. But they also need direction. They may want (and need) to talk about all the places they\u00a0<em>don&#8217;t<\/em>\u00a0want to be, or to go back to, but effective therapy for depression is about so much more than that.\u00a0<sup>(2)\u00a0<\/sup><\/p>\n<p>Just like anyone trying to find their way, a depressed persons needs to develop an ongoing sense of where they\u00a0<em>are<\/em>\u00a0going, how they&#8217;ll get there and what &#8216;markers&#8217; of progress to look out for. Once they&#8217;ve arrived at their non-depressed, happier destination, they&#8217;ll need to know how to get back there again if need be. That&#8217;s why I use these <a href=\"\/blog\/3-easy-psychotherapy-techniques-that-identify-your-clients-real-problem\/\">psychotherapy techniques<\/a>.<\/p>\n\n<h2><strong>Four ways you can help your depressed clients with directions<\/strong><\/h2>\n<h3>1. Establish clear goals<\/h3>\n<p>A <a href=\"https:\/\/www.unk.com\/blog\/why-is-your-client-depressed\/\" data-wpil-monitor-id=\"5\">depressed client<\/a> may (quite naturally) want to tell you all about their depression, but to really begin working with them you&#8217;ll need to discover\u00a0<em>where they actually want to go in positive terms.<\/em><\/p>\n<p>Ask questions like:<\/p>\n<blockquote><p><em>&#8220;Okay, so how are we going to know when you&#8217;ve left that depression behind you?&#8221;<\/em><\/p><\/blockquote>\n<p>They might reply in the negative:<\/p>\n<blockquote><p><em>&#8220;I won&#8217;t be so miserable all the time.&#8221;<\/em><br \/>\n<em>&#8220;Okay, so what will be there instead of misery?&#8221;<\/em><\/p><\/blockquote>\n<p>Get the person to vividly\u00a0<strong><em>describe in positive terms exactly how they&#8217;ll know the depression is gone.<\/em><\/strong><\/p>\n<h3>2. Break down all or nothing thinking<\/h3>\n<p>When people are depressed they are more emotional. The emotional brain has us thinking in very all-or-nothing terms:<\/p>\n<ul>\n<li>&#8220;I am\u00a0<em>completely<\/em>\u00a0ruined&#8221;<\/li>\n<li>&#8220;<em>Nothing<\/em>\u00a0good\u00a0<em>ever<\/em>\u00a0happens to me&#8221;<\/li>\n<li>&#8220;People\u00a0<em>always<\/em>\u00a0let you down.&#8221;<\/li>\n<\/ul>\n<p>One thing I&#8217;ll do early on is describe how <a href=\"http:\/\/www.unk.com\/blog\/3-simple-ways-to-reframe-your-clients-negative-bias\/\">depression seduces people into oversimplified negative thought<\/a>. I&#8217;ll ask people to notice this in themselves and challenge it (&#8220;If you see the Grand Bazaar you&#8217;re going the wrong way!&#8221;)<\/p>\n<p>I might say:<\/p>\n<blockquote><p><em>&#8220;As you begin to leave that depression behind you&#8217;ll start to notice how your thinking becomes less extremist&#8230;&#8221;<\/em><\/p><\/blockquote>\n<p>Depression thrives on black and white thinking. A clear sign that people are coming out of depression is that they&#8217;ll be\u00a0<strong><em>doing less black and white thinking<\/em><\/strong>.<\/p>\n<h3>3. Grade the depression<\/h3>\n<p>Depression can be a scary beast. Because depression has us using less of the strategic &#8216;left brain&#8217;, encouraging a depressed person to be more strategic can help their mood. Asking someone to grade the severity of the depression on a scale helps them to:<\/p>\n<ul>\n<li>break down black and white thinking<\/li>\n<li>notice improvements.<\/li>\n<\/ul>\n<p>So you ask them:<\/p>\n<blockquote><p><em>&#8220;If one is the most depressed it&#8217;s possible to be and ten is the happiest, where are you right now, would you say?&#8221;<\/em><\/p>\n<p><em>&#8220;I&#8217;d say I was a three.&#8221;<\/em><\/p><\/blockquote>\n<p>Now, rather than just &#8216;completely depressed&#8217;, we&#8217;ve got a &#8216;3&#8217;. You might begin to break down the depression even further by asking:<\/p>\n<blockquote><p><em>&#8220;What stops you being a two, or a one?&#8221;<\/em><\/p><\/blockquote>\n<p>The answer may give you clues as to what they need more of in their life.<\/p>\n<p>Or you can go in the other direction:<\/p>\n<blockquote><p><em>&#8220;What do you suppose will be different when you&#8217;re at a five?&#8221;<\/em><\/p><\/blockquote>\n<h3>4. Check the roadmap for avoiding depression in future<\/h3>\n<p>Toward the end of therapy it&#8217;s important to check that they know how to ensure they stay out of depression in the future. <a href=\"\/blog\/how-to-get-over-a-fear-of-depression-relapse\/\">Relapse<\/a> rates for depression are much lower in people who have received effective psychotherapy rather than just drug treatment.\u00a0<sup>(3)<\/sup>\u00a0You might ask:<\/p>\n<blockquote><p><em>&#8220;Okay, so from what you know and have learned, what active steps will you take in future to avoid getting the way you were again?&#8221;<\/em><\/p><\/blockquote>\n<p>The depressed mind feels lost and directionless; a good therapist will act as the friendly, knowledgeable guide &#8211; because we can all feel lost in
